This form is due by Jan 1 each year.

If you are the landlord or owner of more than one park/marina, please complete a form for each.

Oregon Revised Statute (ORS) 90.730

ORS 90.730 requires that every manufactured dwelling facility/marina register annually with the Oregon Housing and Community Services Department.

Facility Name Changes

If the facility name is changed, someone from hcs.mmcrc@hcs.oregon.gov will reach out to request a new W-9 form for your registration.

To prepare for registration, please download the W-9 form from the IRS website: https://www.irs.gov/pub/irs-pdf/fw9.pdf . Complete the form, save it as a PDF, and keep it ready for upload when requested by our team.
